St. Peter's Health Partners is seeking a full-time Nurse Practitioner to join the team covering our long-term care facilities located in the Capital District, New York.

Our stellar team provides compassionate care to seven Nursing Home facilities in the Capital District. This opportunity would be providing care primarily for the residents of Eddy Village Green in Cohoes.

Eddy Village Green at Cohoes is the first of its kind in the greater Capital Region to replicate The Green House model. The facility is made up of 16 ranch style houses, of which each can house 12 individuals. Each home has private bedrooms / baths, and family style kitchens.

Opportunity details:

• Flexible hours, Days position
• Full-Time : open to a shift of 4 - 10 hour shifts or 5 - 8 hour shifts
• On call about 1 in every 8 weeks (Weeklong call, phone calls only)
• Abundance of support services in the facility including providers for Pulmonary, Physiatry, Psych, WOCN, Podiatry and more

Requirements for this opportunity include:

• Board Certified
• NYS License and DEA
• Experience with geriatric population
• Certification in geriatrics a plus, but not required.
• Applicants should possess excellent communication skills, and a strong commitment to providing excellent care that is team oriented
