Providence Hospice of Petaluma is seeking a full-time nurse practitioner to join a compassionate team. This position will be supporting a team of RN's as well as a Hospice physician by performing symptom management, med management, and visits for recertification to end of life patients. The role requires candidates to provide at-home services as well as travel to various long-term care facilities throughout the area. Travel between patient homes and long-term care facilities is required
Bi-lingual skills are a plus

Compensation is between $75.96 and $94.95 per hour
The compensation amounts listed may include productivity or quality/service incentives, call pay, extra shift incentives, as applicable to the position. In addition, providers may have CME benefits available to them.

Providence Hospice of Sonoma County is one of the oldest hospice programs in the country, providing individualized and compassionate care to patients and families throughout the area. This well-established group collaborates closely with Providence Palliative Care as well as partners with local hospitals, assisted living facilities, skilled nursing facilities, and board and care facilities in order to provide full-spectrum care services to some of the most medically complex and vulnerable patients in the county.
